Parental Consent Form for UPDEIT platform and mobile application

Dear Parent/Guardian,

We are excited to introduce UPDEIT, a platform and mobile application designed to assist parents and caregivers of children with developmental delays or disabilities. UPDEIT offers a milestone tracker, which allows you to monitor your child’s developmental progress and provides strategies for working effectively with children facing developmental challenges. In order to ensure the best possible support for your child, we kindly request your consent for the use of this application and the data you input related to your child.

Parental Consent

I, by accepting the terms and regulations of use, give my consent for the use of the UPDEIT platform and application for my child. I understand that the data I input into UPDEIT, including developmental milestones, progress notes, and any additional information, will be stored securely within the application.
