Practitioner Consent Form for UPDEIT Application

Dear Practitioner,

We are excited to introduce UPDEIT, an application designed to assist practitioners working with children who have developmental delays and/or disabilities. UPDEIT offers a milestone tracker and provides strategies for effective intervention and support. In order to enhance the quality of care and support for these children, we kindly request your consent for the use of this application and the data you input related to your clients.

Practitioner Consent

I, by using the application, give my consent for the use of the UPDEIT application for the children I professionally work with. I understand that the data I input into UPDEIT, including developmental assessments, progress notes, and any additional information, will be stored securely within the application.

Purpose of Data Collection

The data collected through UPDEIT will be used for the following purposes:

1. To track and monitor the developmental progress of children I work with.
2. To provide personalized recommendations and strategies for supporting their development.
3. To contribute to the collective knowledge and understanding of interventions for children with delays or disabilities.

Confidentiality and Security

I understand that the data entered into UPDEIT will be kept confidential and will only be accessible to authorized personnel involved in the care of the children. All necessary precautions will be taken to ensure the security and privacy of this information.

Withdrawal of Consent

I understand that I have the right to withdraw my consent for the use of UPDEIT at any time. Withdrawal of consent will result in the cessation of my use of the application and the deletion of any data I have inputted.

Agreement

By using this application, I acknowledge that I have read and understood the terms of this consent form. I agree to the use of UPDEIT for the children I work with and the collection of data as described above.
